    Job Summary:

    Under the direction of the Clinical Nurse Supervisor provides skilled professional nursing care to the patient as prescribed by the Physician and in accordance with the policies and procedures of the Visiting Nurse Home & Hospice.  Gives care in a manner that promotes quality, continuity and safety for the patient and family.

    1. Provides professional nursing care utilizing all elements of the nursing process.
    2. Gives skilled care to patients requiring part time professional nursing service.
    3. Implements orders for medications and treatments as prescribed by the physician.
    4. Administers prescribed treatments, teaches and supervises patient, family members, or others who give such treatments.
    5. Initiates and applies appropriate preventative, therapeutic, and rehabilitative nursing procedures in accordance with Physician's orders.
    6. Observes, evaluates and reports to physician patient's physical and emotional condition, reaction to drugs or treatments.  Interprets to him social and physical factors in the environment that affect patient care.
    7. Instructs patient and family in self-care techniques, diet, medications, as outlined in physician plan of care.
    8. Recognizes and utilizes opportunities for health care teaching with patient and/or family members.
    9. Assists patient with activities of daily living as necessary while promoting the highest level of independent functioning.
    10. Selects patient for care by Certified Nursing Assistants and interprets, demonstrates and supervises the care.
    11. Participates in orientation program for new nurses, other professional staff, certified nursing assistants and student nurses.
    12. Completes necessary admission record forms, maintains daily time sheets, patient/family records, discharge summaries, requests physician certification/recertification and verbal order forms, and prepares other interagency correspondence.
    13. Follows policies and practices of the Visiting Nurse Services.
    14. Accepts assignments and performs duties within the limits of preparation and experience.
    15. Accepts responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service programs.
    16. Applies measures to prevent and control the spread of communicable diseases as required by Rhode Island Department of Health.  Complies with the agency's Infection Control Program.
    17. Provides families with information, support, and encouragement that may help them to adopt attitudes and practices that promote health and reduce anxiety, tension and fatigue.
    18. Helps individuals and families to accept and adjust positively to physical, mental, and social limitations.
    19. Interprets the needs of handicapped individuals to families and gives guidance in the effective use of resources for diagnosis, treatment and rehabilitation.
    20. Consults about individual or family health problems with other professional workers, such as nutritionists, social workers, psychologists, and psychiatrists.
    21. Helps the family to accept appropriate medical, hospital, nursing home, welfare, or other care as necessary. Interprets extent and limitations of service available. Arranges referral and communicates pertinent family information to the agency.
    22. Participates in Health Fairs, Adult Health Clinics and annual influenza immunization programs.
    23. Provides financial assessment forms to family when service need is likely to continue beyond the point when third party will cover the services.
    24. Teaches and monitors family members or significant others, to do prescribed treatments and give nursing care, within the abilities and willingness of the person to learn.
    25. Attends and participates in interdisciplinary team meetings.
    1. Comprehensive knowledge of current nursing practice.
    2. Ability to make meaningful observations and to complete concise, accurate reports.
    3. Thorough knowledge of and ability to interpret agency program and policies to patient/family, and public.  Helps family or significant other accept responsibility for providing care.
    4. Ability to exercise independent professional judgment and to effectively oversee and manage pt. care and function in the role of case manager.
    5. Ability to relate objectively with co-workers, patient/family, other professionals in the agency and in other agencies and the public.
    6. Ability to recognize needs and concerns of people and to deal constructively with them.
    7. Knowledge of and ability to use community resources.
    8. Ability to study and evaluate own job performance and to plan and carry out program for continuing professional growth.
    9. Ability to demonstrate competency in adult phlebotomy.
    10. Maintains current RN licensure in Rhode Island and Massachusetts and obtains necessary CEU's to maintain licensure. 
    Education and Experience
    1. Graduation from a school of nursing accredited by NLN:  from a baccalaureate or masters degree program in nursing or from a state approved diploma or associate degree program in nursing.
    2. Nurses without a baccalaureate degree must have four years experience in nursing. Staff is encouraged to pursue their education toward a baccalaureate degree in nursing.  (This can be waived if determined by CEO.)
    3. Registered as a professional nurse in Rhode Island and Massachusetts.  If Mass license not held at time of hire, employee has 1 year from hire date to obtain.
    4. Interest in keeping abreast of trends and developments in nursing field.
    5. Current certification in CPR.
    Job Specifications
    • Hourly / Non-Exempt / Full-Time (40 Hours)
    • Every other Weekend and every other minor holiday rotation required per department needs.
