Coordinator of Person-Centered Planning
The James L. Maher Center helps people with intellectual, developmental, and other disabilities live life to its fullest – In the ways that matter most to them. That means developing customized programs of employment and volunteer opportunities, creative, social, and recreational activities, and more – in whatever combination reflects our participants’ unique affinities and aspirations – and providing job coaching, transportation services, and other support as needed.
ESSENTIAL JOB FUNCTIONS
· Lead the Maher Center’s efforts to embed person-centered planning in all aspects of participants’ programming
· Promote and advocate for the rights and dignity of all participants
· Support participants in their desire to live their lives to the fullest, using a positive growth mindset
· Look beyond disability labels and diagnosis to identify an individual’s strengths and support needs in a positive way that emphasizes community engagement and integration
· As a professional and positive representative of the Center, develop and cultivate connections between Center and community to promote individualized programming
· Schedule and facilitate effective person-centered planning meetings with a diverse group of Center participants, family members, community allies and professionals
· Synthesize large amounts of information from a variety of sources and the historical record
· Produce well-written, concise, informative reports summarizing person-centered planning meetings
· Support participants’ rights and increase their quality of life while ensuring their voices are heard
· Develop and monitor data-driven protocols and effectively monitor progress
· Produce written meeting summaries given a specific template and timeline
Essential Skills:
· Culturally and linguistically competent
· Familiarity with Microsoft suite
· Ability to manage competing demands and timelines across departments
· High level of organization
· Ability to work independently and with others
· Flexibility with regard to schedule – may require occasional evening or weekend hours
· Ability to listen deeply and respectfully to a diverse group of people
· High level of commitment and reliability
QUALIFICATIONS:
Required:
- BA or BS from an accredited college or university
- Minimum 2 years’ experience working with people with developmental disabilities
- · Excellent Verbal and Written Communication Skills
- Positive Attitude and Role Model for Peers and Subordinates
- · Solution-Oriented, Proactive, Effective Critical Thinking and Strong Organizational Skills ·
- Demonstrated Ability with Microsoft Office Suite
- A valid driver's license in state of residence and favorable National Background Check required
Preferred:
- Experience with the person centered planning process and supported decision making model
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
